Online Appointments for
Primary Care and Optometry may be made via the Internet
at
tricareonline.com .
Beneficiaries must be enrolled in TRICARE Prime or
TRICARE Plus and assigned to a primary care manager at
Keller.
For local technical support send email to
TOL_help@wp.amedd.army.mil. Your email must
contain: the beneficiary's first and last names, the
last four numbers of the sponsor's social security
number, and a brief description of the technical
problem.
Keller's Appointment Center is open from 6 am to
7 pm, Monday through Friday, except for Federal
Holidays. To make an appointment call 845-938-7992 or
800-552-2907.
In the event of severe weather conditions, you may want
to contact the Appointment Center to confirm or
reschedule your appointment before coming to West Point.

TRICARE Young Adult Prime Option available Jan. 1:
TRICARE Young Adult is a premium-based
program that
extends medical coverage to dependents up to age 26.
Most dependents are eligible to purchase this plan
if they
are unmarried and under age 26, are not eligible for
their own employer-sponsored health plan and the
sponsor is TRICARE eligible/covered.
Contact TRICARE for costs or go to the TRICARE
website below. Cost shares/co-pays, deductibles and catastrophic caps vary by program option and the
sponsor’s status.
TYA beneficiaries already enrolled in the
Standard/Extra option who wish to switch to Prime
must submit a new TYA enrollment form and premiums
will be automatically adjusted.
New TYA beneficiaries must submit an enrollment form
and the first three months’ premiums for the
selected option.
Health Net Federal Services must receive all forms
by Dec. 20 for coverage to begin Jan. 1, 2012.
